Your journey starts with a new patient consultation. Our goal is to guide you through your fertility journey, learning about your history, answering questions about treatment options, and getting to the root of your fertility concerns and goals.

Request an Appointment

  • To create a customized fertility treatment plan, our providers must understand your medical history. Once you have scheduled an appointment with us, you'll receive an email to sign up for our patient portal, Prelude Connect. Please complete all of the new patient forms in the portal prior to your appointment, including the electronic medical release form. This will allow us to access your medical records from other healthcare providers.

  • When you arrive for your appointment, please bring the following things with you: 

    • Photo ID
    • Insurance card / insurance coverage information
    • Insurance co-payment or self-payment

    During this time, you'll meet with a provider to review your medical history, discuss your fertility goals, and discuss potential treatment options that may work best for you. This is a great time to ask questions and share any information you think will help us better understand your situation.

    Following this appointment, your provider will devise a customized treatment plan for you as well as order appropriate diagnostic testing for you and your partner (if applicable). A member of your care team will reach out regarding next steps, typically via the Prelude Connect portal.

  • Your care team will work with you to determine a timeline and clarify what you can expect during each stage of your treatment plan. Our team values open communication with patients, and you're always welcome to contact us with questions or concerns.
